  1. Filling the Application
  • The application form must be completed by typing the required information into the Word document. The text fields are expandable as you type.
  • The application form should be completely filled, giving pertinent information according to instructions given.
  • Please do not leave any sections blank and indicate as ‘not applicable’ any questions that are not relevant.
  • The relevant section and page numbers on the proposal must be indicated, where relevant.
  • All documents must have the title and version no as a header and the page number as footer.
    (E.g. For new submissions: Version 1, for resubmissions: Version 2, 3 accordingly)
  • Applications and all other relevant documents including translations should be typed. Handwritten applications /documents will not be accepted.
  • Once filled, the applicant and the other co-investigators must sign on the hard copy of the application form.
  • E-signatures will not be accepted for applicants from Sri Lanka.

 

  1. Documents to be submitted
  • Please fill the checklist and submit the relevant documents
    • One copy of each of the following;
    • Covering letter signed by the applicant
    • Letter from supervisor (if relevant)
    • Bank receipt (if applicable)
    • Curriculum Vitae of All Investigators (if not affiliated with University of Sri Jayewardenepura)
    • Executive summary including study title, investigators, study design and research objectives
  • Two (02) sets of copies of the application form duly filled and signed with the following documents stapled and bound together.
    • The proposal
    • Study instruments in English and Sinhala and Tamil translations where relevant. Information sheet in English and Sinhala and Tamil where relevant. The information sheet in all languages should contain the following statement: “If you require any further clarification regarding this study, please contact the Research Ethics Committee using the contact details provided below, since this research proposal has been reviewed and approved by the Research Ethics Committee, Faculty of Allied Health Sciences, University of Sri Jayewardenepura.   1. Submission Process

 

  • Duly completed application documents, signed by the relevant Heads of Department, should be submitted to the REC Office.
  • A soft copy of all submitted documents, compiled into a single PDF file, should also be submitted via email to rec.fahs@sjp.ac.lk. The email subject line should include the reference number assigned at the time of submission of the hard-copy documents.
  • The REC Office is located at Sagra Building, Faculty of Allied Health Sciences.
  • Office hours: Monday to Friday, 8.30 a.m. to 3.30 p.m.
  • The REC conducts its meetings on the third Monday of each month, except during April and December.
  • To be considered at the next scheduled REC meeting, duly completed applications must be submitted by 12.00 noon on the Monday of the fourth week of the preceding month.
